This case about wernickes encephalopathy is explained with every  fine detail such as few seizures  in the past and talking and laughing to himself. The drugs administered are with high efficacy and the mechanism of action are also well explained. 

CARDIOLOGY

The case about myocardial infarction is well explained and the cause is also mentioned that, he got the pain on the right chest after the administration of covishield vaccine. The drugs used and their action are also well explained.


GASTROENTEROLOGY

The case is about pancreatitis,pseudocyst and broncho-pleural fistula.mentioned also that patient already suffered pain in abdomen in the past and get recovered. Patient also had a burning sensation while micturition.The ct scan and x ray reports confirmed the condition. The condition is very well explained.

This case explains about the liver abcess caused by drinking locally made alcohol, which may be both amoebic or pyogenic.It also mentioned that alcoholism never was an etiological factor of liver abcess.it is also mentioned that the right lobe of liver.  It is easy and understandable.

This case describes about renal failure and its complications. By the way of presentation it is easy to understand the complications of renal failure.It also explains reason behind inyermittent episodes of drowsiness.  This case also mentions about  TURP complications and also mentioned about fleshy mass appearence in urine. By the way of presentation,it is easy to undeestand.

This case about cardiogenic shock in which the patient suffered shortness of breath. This whole thing happened due to decreased preload which inturn lead  to decreased cardiac output. The drugs  used arewith high efficacy. 

PULMONOLOGY

This is a case of COPD, in this case the main cause is due to the obstruction of airways by the dust of paddy fields which acts as lung irritants. The drugs mentioned are also with high efficacy. The other conditions are also mentioned.

This case is about auto digestion of  pancreas.the case is well explained with past history of patient like relapse of vomitings and pain in abdomen after consumption of alcohol. The case is very well explained which helps in understanding.

CARDIOLOGY

This is a case of heart failure,in which patient suffered chest pain with shortness of breath and bilateral carpo pedal oedema. In this case why the process of pericardiocentesis cannot be done is well explained. It is because there is no cardiac temponade. The further development of heart failure and its risk factors are well mentioned.

This is a case of altered sensorium.very fine details of the case are mentioned such as history of seizures in the past, laighing and talking to self, and the development of short term memory loss. Drugs given are with high efficacy and the mode of actions also very well mentioned.

I didn't get any chance till now to make a case report,so I didn't  make any blog to upload in this. 
                             


ANSWER 3 

 A 18 year old male,from miryalaguda,who is a student, came to the hospital with chief complaints of low backache 1 week ago,fever since 5 days ,yellowish discolouration of eyes since 3 days ,vomitings (2 episodes) and loose stools(3 episodes)and blood tinged urine yesterday morning.




positive points:
  •  the progress of the disease was very well described.
  •  the clinical investigations and the pictures helped us to understand the case properly
  •  the history was well taken.




ANSWER 4 

A CASE OF A 45 YEAR OLD MALE WITH ACUTE KIDNEY INJURY ON CHRONIC KIDNEY DISEASE (HYPERTENSIVE NEPHROPATHY) WITH URAEMIC ENCEPHALOPATHY.




positive points:

  •  the history was well taken 
  •  the information was well organised
  •  the clinical investigations name was given along with the pictures of the investigations
  •  the diagnosis was clear
  •  general and systemic examinations were well done


negative points:
  •  the treatment could be more elaborated and made easier to understand.


ANSWER 5

This blog helped me to share my opinion on some of the logged cases. This thing provides us to know more about how the case of a particular condition is studied. This blogging taught me the common mistakes we make which makes the blog less presentable and I can avoid it in the future. This has also taught me how important is history taking and observation and clinical investigations are in giving the right diagnosis.
